Health Examination Requirements

State of Illinois requires students to be excluded from schools if requirements for health examinations and immunizations have not been met by October 15.

Health Exam required for all new students and students entering Grades K & 6.

Dental Exam required for all students entering Grades K, 2, & 6.

Eye Exam required for all students entering K.
